Some of the information in the above document may be out of date. I received the following communication from someone stating they worked for the humane society:

We do not issue licenses for service animals, but we do provide general City & County dog licenses. Please remove reference to our issuing service animal licenses and refer people to the Disability and Communication Access Board for state of Hawaii requirments under both ADA and FHA.
